Talk to the agency

Call or email the office administrator. State the specific issue with dates, times, and dollar amounts. Ask for a written response within 7 business days. Most disputes end here — when agencies know you have records, they find a way to fix it.

Send a formal dispute letter

A formal letter sent by certified mail (or email with read receipt) with attached documentation. States the amount owed, the deadline, and the next steps if the deadline passes. This is the letter that gets forwarded to the agency's legal or compliance team.

File a regulatory complaint

Two tracks depending on the issue. For wage theft (unpaid hours, unpaid overtime, training time), file with the Department of Labor Wage and Hour Division. For billing for shifts that weren't worked, file with the state Medicaid Fraud Control Unit. Both are free and confidential — and both trigger investigations agencies take seriously.
